Laser hair removal remains one of the most popular cosmetic procedures in the country, and for good reason: This non-invasive procedure offers the benefits of minimal downtime and little discomfort—plus, almost anyone is a viable candidate. For many patients, laser hair removal grants them daily convenience; in fact, the most common requests for this procedure in my practice come from women who are tired of shaving their underarms, legs and bikini area or want to be rid of unwanted hair. Many men also seek treatment for back or chest hair and ingrown hairs in the beard area.Unlike long-standing (and somewhat antiquated) methods of hair removal like waxing or threading, laser hair removal attacks the hair in its growth center below the skin’s surface. It targets the hair follicle while it is in the growth phase (scientifically termed the anagen phase). Based on a scientific principle called selective photothermolysis, the laser technology matches a specific wavelength of light to pigment in the hair follicle without damaging the surrounding tissue. Melanin is the primary target, making dark-haired, light-skinned patients ideal candidates for the procedure.The laser affects only active hair follicles. Because the hair grows in multiple phases, several sessions are needed to effectively destroy the hair in all phases of growth. Depending on the area of treatment—and after considering numerous variables, including skin color, the coarseness of the hair’s texture and the cause of unwanted hair—I typically recommend that my patients undergo a minimum of four to six treatments, allowing anywhere from three to eight weeks between each visit. Like any evolving technology, laser hair removal has its limitations. Because the absorption of laser light is selective, blonde, gray or white hair—i.e., hair that lacks pigment—will not respond well to treatment. Patients with light-colored hair may wish to explore a treatment like electrolysis, which uses a micro-needle to transmit electrical energy in an attempt to destroy the hair bulb. A prescription cream called Vaniqa®* (eflornithine hydrochloride Cream, 13.9%) may also be modestly effective in slowing (but not eliminating) hair growth.It’s also important to have realistic expectations. According to the FDA, permanent hair reduction is defined as a long-term, stable decrease in the number of hairs that re-grow after a treatment regimen. Although laser hair removal will permanently reduce the total number of hairs in the treated area, it may not result in the removal of all hair. Many patients require annual follow-up appointments to maintain their desired result.Recent advancements in laser technology have further improved the capabilities of laser hair removal. Pre-existing laser technology has been difficult to use on darker skin tones, as it lacks the ability to differentiate between melanin in the skin and hair and may cause skin discoloration in darker complexions. New laser wavelengths and skin-cooling devices are now capable of discriminating between hair and skin pigment, allowing us to safely treat darker complexions without burning the skin.As with any cosmetic procedure, laser hair removal is technique-dependent. Look for a qualified physician who is board-certified in dermatology or a related specialty, and stay committed to maintenance treatments to ensure the best—and most fuzz-free—results.

It’s no secret that regular use of sun protection not only exponentially decreases your risk for skin cancer, but will also keep your skin smoother, healthier and younger-looking over time. Yet one of the biggest misconceptions about sunscreen is that it isn’t necessary aside from, say, the beach or pool. When patients ask me whether they need to wear sunscreen during the winter, I always respond with an emphatic “yes.”First, it’s important to understand the difference between UVA and UVB rays. UVA rays—the kind that penetrate the skin and promote collagen breakdown—remain just as strong throughout the winter months (and even on cloudy days), so they can still cause photodamage and an increased risk of skin cancer. This means that daily use of sunscreen is a must to protect against UVA rays. Though UVB rays, known as the “burning rays,” tend to be stronger during the hot days of summer, they are capable of causing sunburn any time of year. In fact, the sun’s rays can be up to 80 percent stronger when reflected, and because snow causes a reflection, skiing or sledding—or even being around glass buildings in a metropolitan area—can lead to a sunburn without proper protection.I recommend a sunscreen with an SPF of at least 30 for daily use and one with SPF 30 to 50 if you’ll be outdoors. I tell my patients not to be overconfident when using a high SPF and to reapply frequently, especially if the skin gets wet. Since the skin tends to be dryer in the winter (think less humidity and buildings with cranked-up heat that dries out the skin), it’s also important to moisturize regularly, drink lots of water, and invest in a humidifier to keep skin moisturized. Taking an omega-3 supplement daily is also a smart way to keep skin supple.Windburn can also wreak havoc on already-dry winter skin, as exposure to strong, cold winds can strip the skin of its natural oils and protective moisture barrier. Look for products that contain aloe or hyaluronic acid, which draws moisture to the skin to help restore the lipid barrier.My best advice: When it comes to sun protection, treat every day like a sunny day. Think of sunscreen as a 365-day-a-year requirement. Your skin—and your health—will thank you.

The old adage “you are what you eat” has stuck around for a reason: Poor diet has been linked to disease, obesity and decreased energy, while good nutrition has been shown to have innumerable health benefits. When it comes to the skin, research also shows that making nutritious choices can directly affect your complexion by fighting the signs of aging and improving skin conditions like dryness and acne.Antioxidants are all-stars in skincare products—not only do they protect skin from free radicals, but they help counteract the breakdown of collagen. Eating a diet rich in antioxidants can also support overall skin health. A study has shown that taking antioxidant supplements (with a combination of minerals and glycosaminoglycans), like coenzyme Q10 or vitamin E, can help improve skin roughness and fine wrinkles.1 Eating antioxidant-rich foods can have a similar effect, think cruciferous vegetables (the darker, the better), citrus fruit, berries, pomegranate, grapes, red wine, nuts, and green tea. In addition to drinking plenty of water, eating foods with high water content like melon or cucumbers keeps the skin and body hydrated. A daily omega-3 supplement, like fish oil, can also help decrease inflammation and moisturize the skin from within, but make sure you check with your doctor before taking any new supplement. Although the “chocolate causes acne” myth has lost momentum in recent years, there may be something to it after all. Research has shown that excess sugar intake can lead to increased insulin production, which in turn causes greater oil production in the skin, so stay away from refined sugars if you’re acne-prone.2A recent study conducted at the University of Nottingham showed that a diet rich in carotenoids (found in tomatoes and carrots) can help to improve skin tone and give the skin a healthy glow that mimics the look of a suntan—all the more reason to skip the tanning bed!3A smart rule of thumb is that if something is good for the body, it’s also healthy for the skin. Our skin reflects what we put into our bodies, so feed it well.

How Realistic are our Expectations? - Skin Medica Beauty Talk

The list of our expectations from skin care products and procedures is l-o-o-o-n-g. We want wrinkles and brown spots to disappear, skin become tighter and firmer, dark circles and puffiness under the eyes – GONE! How reasonable are our expectations? When I start patients on a new skin care regimen I want them to be excited about it, but at the same time, I educate patients about WHAT they should expect from their skin care regimen, and WHEN the result will become noticeable. Expecting perfection is not realistic, expecting improvement is. Just as the book “What to expect when you’re expecting” tells a pregnant mother what to expect at each stage of her pregnancy, I too take the same approach when recommending skin care regimens or procedures to my patients. An informed patient is always a better patient!In order to have realistic expectations about their prospective improvements, patients need to understand what processes are taking place within the skin at each step during the course of the treatment, knowing that, these processes will take TIME. I often have to remind patients that it took blah-blah years to accumulate the skin damage, so it will take some time (and commitment) to fix it. Repair is never as good as prevention, particularly with sun damage. Depending on the patient’s pre-treatment skin condition, the time and the degree of improvement will vary substantially. It is clinically proven that skin improvement from anti-aging products that contain antioxidants and growth factors, as well as laser treatments and chemical peels, require certain time to build up. For example, laser treatments and chemical peels initially create inflammation in the skin, but as the skin heals, a number of beneficial changes occur. The first signs of improvement, such as more even skin color, smoother texture, and reduction in hyperpigmentation, can be seen as soon as 2 weeks after the procedure. On the other hand, the changes within the skin, dermal regeneration, such as: collagen stimulation, re-orientation of new skin connective tissue and skin tightening can take several months. Keep in mind, these changes are gradual; they will not appear over night. Since we are looking in the mirror (sometime magnifying mirror) every day in anticipation of improvement, those steady changes often are not that easy to see. I always take pre-treatment photographs, which help me objectively evaluate the patient’s progress, and also give the patient a chance to compare. The improvements seen from anti-aging skin care products are even more gradual than with procedures. To see the effect of the product, you have to use it for at least several weeks, not just a few days. Ask yourself: have I given it enough time for the product to work?The answer to better skin is not a “magic potion”, but rather a scientifically-based approach to addressing your skin care concern. When paired with compliance and patience, people are sure to start complimenting you on the appearance of your skin and then you will know it was all worthwhile.

Richard Fitzpatrick, M.D.; Dermatologist, GBK Cosmetic Laser Dermatology, San Diego, California; Founder, SkinMedicaI met my first laser in 1962 as a freshman at Princeton University, and as soon as I started playing with the then-brand-new argon laser in physics class, I was instantly fascinated by the fact that you could concentrate a beam of energy so perfectly. Although no one had any idea how to use this innovation at the time, I immediately saw the potential for such a device, and vowed to work with it some day.In the late 1970s, chemical peels and dermabrasion were popular anti-aging treatments, but they were fraught with problems—the main issue being that they were so uncontrollable. I began to look at the carbon dioxide (CO2) laser and eventually developed the protocol for skin treatment, and from there much attention began to focus on laser skin resurfacing and rejuvenation. Among the innovations that stemmed from the principles associated with my work with the CO2 laser were the use of the erbium laser for skin resurfacing and the whole field of non-ablative, no-downtime laser treatments for improving texture and tightening skin. But the innovation that really started today’s laser revolution as we know it was the fractional laser. By taking original laser treatment principles and delivering the energy in pixels as opposed to all in one spot, we are now able to do things with light-based energy that we weren’t able to do before in a much safer manner.With today’s menu of laser offerings, we can smooth surface texture, eliminate premalignant cells, address redness caused by broken capillaries, tighten skin and improve wrinkles. Since all of these skin concerns are associated with aging, combining different laser modalities can have a profound anti-aging effect.Although lasers haven’t replaced the need for surgical procedures that eliminate excess skin (yet), the two work hand in hand to obtain optimal anti-aging outcomes. The main thing that lasers can do that surgery cannot is improve the quality of the skin, and no matter how good a surgeon is, he or she cannot reverse the damage on the surface of the skin that speaks volumes in terms of visible aging. Even more, we’ve found that laser resurfacing can actually improve the longevity of surgical results. The beauty of laser treatment is that just about everyone is a candidate, although some skin types do require greater expertise. Someone with fair skin is an “easier” candidate since there’s minimal pigment to complicate treatment, but dark skin can respond very well in the hands of an experienced doctor. Anti-aging results from laser treatments are long lasting, provided the patient takes the proper steps to protect their skin from further damage. With diligent use of sunscreen and skincare products, improvement can last up to 10 years. The key is using clinically tested (and proven) ingredients such as growth factors, retinol and antioxidants. Growth factors maintain results by stimulating the skin to repair the damage caused by incidental sun exposure (more than 5 minutes of sun each day incurs too much damage for the skin to repair itself naturally). Retinoids help eliminate irregular cells, keep pigment in check, and stimulate new collagen, and antioxidants shield skin from free radicals that cause aging changes. Vitamin C is an especially important antioxidant since it’s also necessary for collagen production, which is a primary way that laser treatment helps improve the appearance of the skin. In fact, laser treatments prompt the skin to produce new collagen for up to a year after the treatment is performed. While the anti-aging benefits of laser treatment for the face are indisputable, many patients don’t realize the potential improvement they can get on other areas of the body. The neck, chest, arms and backs of the hands can see profound improvement as well, which goes a long way to “match” the rejuvenated skin on the face—and deliver an overall more youthful appearance.

But collagen gets a lot of attention in terms of the aging process, and its decline is the main culprit for fine lines, wrinkles and furrows.Collagen is abundant in young skin, and its rate of production is able to keep up with the rate of its degradation until about the age of 25. At this point, collagen begins to be destroyed faster than it can be replaced, which leads to telltale signs of aging such as fine lines, wrinkles and sagging.
